Contact Form and Mobile Messaging Program
When you fill out our contact form, your information, including limited health-related details, is sent to our Customer Relationship Management (CRM) system and analytics software. This integration helps us manage our interactions with you more efficiently and improve our services. The information is treated as confidential and is protected in accordance with HIPAA guidelines. It will not be shared with third parties without your explicit consent.
By providing your mobile phone number or sending us the word START, you voluntarily opt into our Mobile Messaging Program and agree to receive recurring mobile messages from us at the number provided, even if it is registered on any state or federal “Do Not Call” list. Such messages might include onboarding assistance, transactional messages such as appointment reminders, and informational messages about our services. Messages may be sent through an automated system with a maximum of 4 informational and up to 10 transactional messages per month during onboarding, plus 1-2 transactional messages per appointment. Standard data and message rates may apply from your mobile provider.
You confirm that the number you provide is valid and that you are its owner or authorized user. If your mobile phone number changes or you are no longer the owner or authorized user, you agree to promptly notify us at firstname.lastname@example.org. Participation in our mobile service is voluntary and not required to receive our ABA services.
User Opt-Out and Support
To opt-out of the Program and stop receiving mobile messages from us, reply with STOP, QUIT, CANCEL, OPT-OUT, or UNSUBSCRIBE to any mobile message. Opt-out processing may take a short period, and you may receive a one-time confirmation message. To re-enroll, email us at email@example.com or text START to our phone number. For support, reply HELP to any mobile message. We are not liable for failing to honor opt-out requests that do not comply with these terms. We may change our phone number or short code and will notify you of any such changes.
Disclaimer of Warranty and Liability
Our mobile service and CRM/analytics integration are provided “as-is” and may not be available in all areas or on all mobile providers. We and our service providers are not liable for any failed, delayed, or misdirected delivery of any mobile message or data.
We collect limited data from public databases, marketing partners, social media platforms, and analytics sources during your visit. This includes your location, computer system, pages visited, time spent per page, IP address, country, and potentially social media profiles and referrals.
We do not share your data with any third parties, except as required for spam detection or as legally obligated.
Comments and their metadata are retained indefinitely for follow-up comments. For registered users, we store personal information provided in our CRM .
Your Data Rights
You can request an exported file of your personal data held by us, including data provided by you. You can also request erasure of your personal data, except for data we are obliged to keep for administrative, legal, or security purposes.
Visitor comments may be checked through an automated spam detection service.
We implement robust security measures to protect your data, in compliance with HIPAA and CTIA guidelines. This includes secure data transmission, storage encryption, and access controls.
We may modify or cancel the mobile service or its features at any time, with or without notice. We may also modify these Mobile Messaging Terms, with changes taking effect upon posting on our website. Your continued participation constitutes acceptance of these modifications.
